Here's what we're going to make, or at least replicate:
Step 1: Make a new document 390 x 140 in size, and fill it with black.
Step 2: Paste your render into the middle.
Step 3: Duplicate your render 5 times, so that you have 6 renders.
Step 4: Here is how your renders should be laid out:
Layer 1 - Normal 100% - Gaussian Blur 5 px
Layer 2 - Overlay 50%
Layer 3 - Overlay 100%
Layer 4 - Soft Light 100%
Layer 5 - Soft Light 100%
Layer 6 - Normal 50% - Desaturated
That's going from the original, Layer 1, to all the duplicates, Layer 6 being the last duplicate. If the render is too dark, I suggest getting a normal circle brush, with the colour on white and the mode on soft light, and going over one of the layers until you're pleased.
Step 5: Add the texture below on Screen - 36% opacity. Make sure it's at the top of the Layers menu, above all of the renders and the background.
Step 6: Add the texture below on Multiply - 27% opacity. Again, make sure it's at the top of Layers menu, above all other layers.
Step 7: Add the texture below on Overlay - 100% opacity, once more above all of the other layers.
Step 8: Add the texture below on Dodge - 100% opacity, above all of the other layers.
Step 9: Add a border and some text. I went with a 1px black border and then a 1px white border, and the font I used was Angelic War, which you can get from sites like dafont.
